The spec contains cross-references to hard-coded paragraph numbers, which is problematic, since paragraph can change. For example, the Effects clause in [istream.unformatted]/2 references "~\ref{istream.unformatted}, paragraph 1". Run grep " paragraph " *.tex to find all occurances.
